I carry:

Fully loaded w/ one in the chamber (mag +1)
My carry gun has no decocker, it is DAO. It has no integral safety, either. It doesn't need one any more than a revolver does.
It is loaded with JHPs.

I carry a spare mag as well. It is fully loaded with JHPs.

If I carry my revolver, I have a reload handy in a dump pouch. If I carry a semi auto, I don't top off the mag and a carry a spare mag in a belt pouch.
