How was the Tokarev intended to be carried?

Agreed. It was designed as a military issue pistol. The half cock was set up to prevent a fully cocked pistol from firing if dropped, which was a definite possibility on a battlefield. Supposedly the Russians tested it by dropping fully cocked pistols from the top of a three story building. They couldn't get the fully cocked pistol to fire.
A lot of folks now use that half cock as a manual safety when using the pistol for CCW.

If you are going to carry it without a round chambered for self defense make sure you practice racking the slide to prevent misfeeds.

And don't be gentle when you rack it.

Rack it like you want to break it.

I always liked the looks of the Tokarev but I think I'd be more comfortable with a Makarov.

The Tok might still be a fun range gun. Are the Tok's "import safeties" trustworthy?

I carried a Makarov for some time. The only thing I would be careful about while carrying it is that you may forget you have it with you.
