Well, took my USP .45 to the range today. Fired 200 rounds of CCI Blazer 230g with not so much as a hiccup. I think the extremely dirty extractor and firing pin was the culprit. It is now off it's second 200 round probation period.
Off topic:
The guy next to me let me run a few rounds through his SIG 229 in 357SIG. All I can say is WOW! Recoils less than my G31 and G19!!! I put 5 rounds (sort of rapid fire) into a group that you could cover with a quarter. I now know what my next purchase will be.
